Admission - What to Bring

Following is a list of items that are helpful to have when your child is being admitted to the Hospital:

Paperwork and Important Numbers
  • Referrals
  • Physician names, addresses, phone and fax numbers, e-mail addresses
  • Medical history and medications
  • Government-issued photo identification (ID) card for parent(s) (Drivers license, passport or other official ID)
  • Custody or divorce papers, when needed, to verify who can sign for your child's medical tests and treatments
  • Social security numbers of patient and parent(s) or guardian(s)
  • Employer ID for patient, parent(s) or guardian(s)
  • Emergency contact numbers
  • Insurance information (including medical, pharmacy, and dental insurance cards)
For Comfort
  • Books, movies, puzzles, water, snacks, and toys
  • Comfortable clothes
  • Pajamas and slippers
  • A small number of comfort items (such as a blanket, or toy, smooth surface and washable items recommended, photographs of family, friends or pets)
  • Camera
  • Toiletries
  • Special diets/formulas
  • Cooler for breast milk

Any electronic device brought to the Hospital will need to be approved by our biomedical services.
